The entry form was available in two parts; you may still view the files. One file contains the rules. The other part was the entry form that entrants filled out to accompany their entry slides as well as a permission form for particpation in the Collectors Choice program [details about this program are on this page]. To print the entry form and rules, use the printer icon on the Abode Reader bar that is right above the document.
To access and print the forms, you will need the Acrobat Reader, version 7.0 or later. It is free and can be obtained by clicking on the Acrobat icon. If you have an earlier version of Acrobat Reader, you can also update it by clicking on the icon.
