The entry form contains the rules as well as a page you can print out and return with your entry. To print the entry form and rules, use the printer icon on the Acrobat Reader bar that is right above the document.
To access and print the forms, you will need the Acrobat Reader, version 8.0 or later. It is free and can be obtained by clicking here.
Clicking on your chosen options from the table on this page will take you where you need to be.
You may, of course, also obtain a printed entry form. This includes the rules and regulations for the competition as well as the document that will be completed and sent to the Dairy Barn along with your entry CD.
You may request the printed form by sending a stamped self-addressed #10 business sized envelope to QN '09 Entry Form, c/o Dairy Barn, PO Box 747, Athens OH 45701, USA.
INTERNATIONAL ARTISTS should include a complete postal address and two International Reply Coupons. Please be sure that the postal agent who sells you the coupons stamps the coupon with date and place of purchase. If it is not possible to get International Reply Coupons, please send a note to this effect to QN@dairybarn.org. International artists do not need to send a return envelope.
